The health care professional orders the device from one of our preferred partners and submits the claim to the insurance company, you pay the HCP co-pay/co-insurance when the HCP implants the device.

The Eversense CGM System will be processed and distributed by an in-network Specialty Pharmacy utilizing your pharmacy benefit. You will receive a call from a preferred partner to discuss any required co-pays. The pharmacy will ship the Eversense CGM System directly to the HCP inserting the Eversense Sensor. The HCP and the pharmacy will each submit claims and collect any required co-pay from you.

The DME supplier is determined by your insurance company. You will receive a call from the DME supplier to discuss coverage and any potential co-pay. The supplier will ship the Eversense CGM System directly to the HCP inserting the Eversense Sensor. The DME and HCP will each submit claims and collect any required co-pay from you.
The Eversense E3 CGM System is covered by Medicare for insulin dependent patients who meet the Medicare coverage criteria. If you're on insulin you might benefit from Eversense.
